Outreach Community Health Worker – ACC-JPS 

The Outreach Community Health Worker (CHW) serves as a patient advocate and liaison between the patient/family and community partners to the residents of Tarrant County. This role also performs a variety of clinical and technical functions in patient care. The Outreach CHW will assist clinic leadership in all aspects of patient flow while maintaining a high quality of patient care in a compassionate manner. This job is also responsible for collaborating with community and civic leaders about Community Outreach Programs and JPS Health Network services and programs.

The Diabetes Peer has a deep understanding of their community and is a cultural mediator between their church/faith community and the Baylor Scott & White Health system. They work closely with Community Health and Diabetes Educators to provide basic diabetes and health education, informal counseling and self-management support that is culturally appropriate to their specific setting.

Community Health Workers (CHW) build trusting relationships with community partners and individuals in targeted populations in order to increase utilization of Cancer Care Services’ programs, as well as increase equal access to cancer-related care. CHW’s work closely with health care providers, social service agencies, the faith community, civic leaders, and other groups that serve at-risk populations.
