Community Health Worker - Maximus

Maximus is hiring Community Health Workers (Sr CSR – Outreach) to work with families eligible for Medicaid to educate, assist and enroll in Medicaid Managed Care and learn about preventive health care in Texas Health Steps. Jobs are available for people familiar with these communities in Texas and are Work from Home in the community you serve. Day travel for in person Home Visits and Community Event attendance are required.

Job detail includes attending enrollment and health events, conducting home visits and presentations to promote and educate STAR, STAR+PLUS or STAR Kids and Texas Health Steps. Outbound phone calls conducted prior to home visits. Mileage reimbursed. Mobile phone issued.

These are full-time positions with a flexible schedule to attend events and conduct recipient outreach. Positions are work-from-home with regional offices based out of Abilene, Arlington, Athens, San Antonio, Houston.

Aide, Community Service, WIC - Public Health

Summary: Assist with infant mortality prevention and WIC outreach in the community. 

Minimum Requirements: Certification as Community Health Worker required or, in lieu of the certification, proof of at least 1000 cumulative hours of community health work services within the most recent three (3) years. One (1) year of experience as Community Health Worker. Two (2) years of WIC experience. Organizational and customer service skills required. Must have interpersonal skills to relate well to customers and other agency personnel. Current and valid driver license is required.

Community Health Worker - Equality Health, Dallas

The Community Health Worker (CHW) is primarily responsible for providing supportive care and assistance to a diverse patient population to identify barriers to self-care and healing, decision making and communication in navigating the healthcare system. This individual works closely with Registered Nurses, Chaplains, Social Workers, Care Specialists and other members of the Care Management team, and consistently demonstrates a high standard of ethical behavior, professionalism, sensitivity and confidentiality. Community Health Worker (Bilingual English/Spanish) - Los Barrios Unidos

The Community Health Worker is responsible for managing programs in the community that promote, maintain, and improve individual and community health; and assists individuals to adopt healthy behaviors.

JOB FUNCTIONS (summary)
- Plans, organizes and/or facilitates classes in one or more areas of health (e.g. Asthma, healthy living, Diabetes, Centering Pregnancy, Centering Parenting, etc.).
- Interprets, translates, and/or provides cultural mediation related to health services or information for patients and community members.
- Refers patients or community members to different needed health services. MINIMUM

QUALIFICATIONS
· Education: Minimum High School Diploma or equivalent.
· Specialized Training/Certifications/Licenses:
· TX DSHS Community Health Worker (CHW) Certification; or must be able to obtain           within six months.
· Current Health Care Provider CPR Certification; or must be able to obtain within six          months.

The Community Health Worker (CHW) is responsible for assisting in delivery of services that address the social determinants of health, and bridging gap between communities, individuals and providers. Conducts community outreach and education.

Experience:
Must have one (1) year experience working in a multi-cultural community setting
Oral fluency in Spanish is preferred.
Knowledge of medical terminology is preferred.
